Allow showing custom open commands without enabling the shell extension menu

Hi,
I customized open commands in Advanced/custom_open_commands, but the custom menu only shows up if I set Advanced/context_menu_shell_extensions to true.
I think the custom open commands shouldn't be affected by this option since it's not really a shell extension. It would be great if you could disable the shell extension menu and show a custom menu.

The next alpha update will show the custom open commands in the fallback context menu.

Everything 1.5.0.1408a will now show the custom open commands in the basic context menu.
