I had a handle leakage issue for some time now but I refrained from reporting it as I wasn't sure if the problem lied on E itself or some preview handler(which still might be the case).
Anyway, I restored the default value and the leak problem seems to have gone away. I was regularly noticing E at 200.000+ handles, in one case reached 1.000.000! The number was probably raising whenever media files were changing, not just by me trying to preview them.(which is strange, does this setting has any effect on how properties are gathered? )
I had the default value changed for some past problem with the windows standard Windows Media previewer which I cant recall, but now, everything is working fine with the default value, so all good here.
I think the problem is reproduceable if you scroll some media results up and down and you constantly hit refresh with the preview_multiple_handler_instances=1, the handles number will start raising and will never release anything.

Using the stock Windows Media Media preview host (wmprph.exe)
Neither Everything or wmprph.exe leaks handles when changing the preview, spamming Ctrl + Space, spamming F5 or spamming writes to the mp4 file.
Maybe something else is causing the handle leak in Everything.
Are you indexing any properties under Tools -> Options -> Properties?
preview_multiple_handler_instances only applies to previews.
Is there a particular file type causing the issue (eg: .avi) ?
If you see the issue again, could you please check the opened handles with Process Explorer.
If the selected file is modified in the background the preview is automatically refreshed in Everything.
