Highly fragmented file= $Extend\$UsnJrnl:$J

Highly fragmented file= $Extend\$UsnJrnl:$J

I am using WinXP and my defragger finds this file is highly fragments with 80 parts:
$Extend\$UsnJrnl:$J
It seems to be something to do with Everything. Should I be worried and what can I do about this?

The USN Journal - the file you mention is where it saves it's data - is a part of Windows to keep track of changes to the disk (one USN journal per volume).
Everything reads Windows' USN Journal for updating the Everything index.

You should not be worried.

This file does not get very defragmented in normal use. However when I turn on a setting in Everything it seem to get fragmented very quickly.

I am thinking of the settings "Enable USB Journal" or "Load Recent Changes". I haven't determined which of these two makes the difference.

Would a fragmented USN journal also affect other operations in Windows, separately from Everything?
