Everything doesn't use the string "Offline Disk", this string comes from the OS from a call to GetVolumeInformation . Do you currently have a volume mounted at O: that is online? Unfortunately, I did not do backup Everything.ini - I just Everything.db, I took GUID from the registry.

Everything 1.4.1.746 defaults to auto_remove_offline_ntfs_volumes=1 I'll change Everything from overwriting previous settings. Everything can not see the update, probably since version 1.3 comes with a stable and beta versions.
In the .ini I have set: beta_updates=1 - even though this is the message "Everything is up to date!"

Recent changes are disabled in 1.4.1, you will need to enable recent changes from Tools -> Options -> Indexes. The reason for disabling recent changes is because date modified is indexed by default. Indexing only one of recent changes or date modified is really necessary, you can still have both. R...

Maybe these are hidden files from within Explorer can not see them.

What format is the D: drive NTFS or FAT32?
I suspect that FAT32, because the format is not automatically monitored, if so, click the Folders tab "Update now" or Indexes "Force rebuild"

Download 1.4 portable - run, make sure the work is what you want, then set the options according to their preferences - close Everything and using the file comparison or this page, check both ini and find the difference.

Default Everything indexes filename, path and attributes - so that what you are not interested in the attributes, but perhaps not so bad.

Yes, I do exactly as you wrote.
HashMyFiles says that the two files are identical, and HDDB displays hashes 00015421 and 00015422 and the in column "Duplicate Count" all zeros.
Also hashes of HDDB are useless.
